On the day of check-up, administrative attendants verified that the questionnaire was completed, and asked to complete the questionnaire during the check-up program in case of defaulters. The answers in the survey sheet were recorded through an optical character recognition system. The questionnaire assessed the following variables. (1) Smoking status We used the question, ""Have you smoked more than 5 packs of cigarettes (equals to 100 cigarettes) in your life time?""
